We hear a lot about joints when it comes to osteoarthritis, but far less about what is happening further down the body, in the gut. Yet a recent clinical trial has delivered concrete proof: a daily prebiotic fiber supplement can significantly reduce knee arthritis pain while also improving muscle strength and pain sensitivity. In just six weeks.

As an osteopath, I always pay close attention to what whole-body science teaches us. And this study perfectly illustrates why I never limit my focus to just the painful joint during a consultation.

What exactly is a prebiotic?

Before diving into the results, let us cover the basics. Prebiotics are not live bacteria (that would be probiotics). They are fibers that we cannot digest ourselves, but that our gut bacteria "feed on" to thrive and produce beneficial molecules.

  • Pain sensitivity changed: the threshold at which pain was perceived increased, meaning the nervous system was responding differently

That third finding is particularly interesting. This is not just a mechanical effect on the joint. The brain and nervous system's pain-processing network appears to be influenced by the state of the gut microbiome.

The gut-joint-brain axis: a clinical reality

Why would the gut affect knees and pain? Because the gut microbiome, the vast community of bacteria living in your digestive tract, produces molecules that travel through the bloodstream and influence inflammation throughout the entire body.

When the microbiome is out of balance (a state called dysbiosis), pro-inflammatory substances can leak into circulation and worsen conditions like osteoarthritis. Conversely, a rich and diverse microbiome tends to produce short-chain fatty acids that dampen inflammation.
